Appointments:
Appointments can be made
by calling any of our office locations during our regular
office hours. A receptionist will speak with you to make
the appointment. We shall make every effort to make an
appointment with the physician or practitioner of your
choice.
Same day appointments or
walk-in patients are welcome, but for sick visits only.
Physicals or well child checks need to be scheduled. Same
day appointments or walk-in patients may have to see the
first available physician or practitioner.
Our After Hours clinic is
for sick patients only. If your child needs a physical,
immunizations, or a recheck, please call during our regular
office hours to schedule these. Appointments are not necessary
for After Hours Care.
Please be courteous. Please
try to come at your appointed time. It is difficult to
stay on schedule if several patients come in at one time.
If you do run late for an appointment, we will make every
effort to fit you in. Please call us within 24 hours if
you cannot keep your appointment. Your fellow patients
will appreciate the courtesy, and so will we.
Phone Advice:
As a special service to our patients, we make available
one of our experienced nurses or medical assistants to
offer telephone medical advice under the direct supervision
of our physicians and practitioners. Should your child’s
problem suggest the need for an evaluation, the nurse
will schedule an appointment for you. Telephone medical
advice will be given in the order your call is received.
Urgent calls will be handled immediately. If you have
an emergency, you should call 911.
Medication Refills:
The easiest way to obtain a medication refill is to call
the pharmacy where you purchased the medication and request
for the pharmacist to call us for an authorization. Certain
restricted drugs, such as Ritalin, Adderall, etc., require
a new handwritten prescription. Therefore it cannot be
renewed by telephone.
As a general rule, we do
not renew prescriptions for antibiotics unless the need
to do so is documented in your child’s medical record
as part of an ongoing medical treatment plan. Any new
illness requiring antibiotic therapy also requires medical
examination.
Answering Service:
Our answering service is available for you after our clinics
are closed. You can access this service by calling 386-752-0906.
You will then be directed to the on-call physician for
the practice. If you have an emergency, you should call
911.
Medical Records:
A copy of your child’s records is available to you
(custodial parent or guardian) upon request. You will
be asked to sign a records release before they can be
released to you. The first copy of your child’s
medical records is free. Subsequent copies are $1.00 per
page. Please allow us 5 business days to prepare your
child’s records.
Insurances and Payments:
We are participating providers for most insurances. We
are required by law to collect all deductibles, co-payments,
and payments at the time of service. We do offer payment
plan options and sliding-fee-scale plans. Please contact
